The following documents must be submitted by the Contractor to the Carmeuse Project Manager prior to the start of any on-site work.

 A signed copy of the Commitment Agreement from the H&S Manual (appendix I)

Provide current WSIB Workplace Injury Summary Report (WISR) including both Non Lost Time, and Lost time injury frequency information.

Note: Carmeuse is committed to working with safe Contractors. To meet this objective, Carmeuse has
adopted the WISR format to help find Contractors with effective safety programs. Before a contract
is awarded, your WISR performance will be compared against the industry using the information
provided.

Provide current WSIB Workplace Clearance Certificate. (MUST STATE N/A IN BOX IF NOT APPLICALBE)

Where WSIB Insurance Exemption applies, must provide “letter of exemption” issued from WSIB, agree to, and sign the WSIB Exemption Agreement (appendix II)

 A completed Pre-Qualification form (for all capital projects over $25,000) (Notify Corp Purchasing for most current form)

Provide a copy of Certificate of Liability Insurance naming “Carmeuse Lime (Canada) Limited as additionally insured, with a minimum of $2 million liability
